Wise Wildcats Application

WISE Wildcats supports first-generation college students from low-income backgrounds in building financial stability and independence through coaching, workshops, and connection to resources. Participants will be provided a housing stipend contingent upon their completion of program requirements. This application will help us understand your background, goals, and readiness to engage. Please answer thoughtfully to increase your chances of being selected. 

Important Note – Your responses will be protected in a secure University database that will
be visible only to Basic Needs staff. Student’s personal information will be kept confidential as outlined in FERPA and HIPPA.
Section 1: Basic Information
Note: Your student ID is used to identify you for the purposes of this form, meaning you do not need to enter your legal name or other identifying information. Your student ID number can be found on your WildCat Card or in the Chico State portal.

If you are unable to locate your student ID or urgently need support please call the Basic Needs Office at 530-898-4098 and we will assist you with finding your student ID number and completing this form.

Wise Program Agreement
This program has four key components which are all required to receive your full housing stipend. All participants will co-create and sign an individualized WISE Program Agreement during the program kickoff (summit). Please read the program requirements and
community agreements carefully before signing up for this program – if selected for this program, your housing stipend will depend upon completion of these activities!
